Flu immunisation 2024
We are aware the NHS link to their on-line consent form is not working, but the QR code is so please can you use this (see link below to the letter for more information and to access the QR code). Alternatively, you can print and complete a paper consent form and return that to the class teacher/teaching assistant (link below). If you need a paper copy of the form, please ask at the classroom when you drop off/collect your child, or send a note with your child to ask for a copy and we can send a form home.

Flu immunisations will be taking place in school on Thursday 5th December 2024.
In order for your child to be given the free nasal flu spray, you need to give your consent on-line. The link closes at 9am on 2nd December 2024 so please complete it ASAP. Thank you.
